Our rules are simple! We require you to have your teeth cleaned twice a year, and we require you to keep your appointments. You’re allowed to have your teeth cleaned as many times as you would like (no more than once a month please), and we’ll provide one set of diagnostic xrays (usually 4 “bite wings”) to detect cavities between the teeth twice a year and 1 annual panoramic (or full mouth series) xray to examine the condition of your roots and bone and to look for any possible infections or cancer. The enrollment fees must be paid prior to any restorative work, and all fees for services are required to be paid as services are rendered unless prior arrangements are made with the office.
Your membership entitles you to treatment and services at any of our locations and we are dedicated to providing emergency care in a timely fashion should the need arise… usually within 24 hours. We reserve the right to discontinue your membership without refund should you fail with your routine cleanings or miss multiple appointments. We require that you contact the office at least 24 hours prior to a scheduled appoint if you need to reschedule to avoid a failed appointment charge of $25. There is no limit on the amount of benefits used or an additional annual fees. There is no limit to the number of family members enrolled under a family membership plan, but all members must reside at the same address to be considered a family. Children over the age of eighteen living at a different mailing address than their parents and who are not attending college are to be considered as a separate membership and are subject to associated costs of membership.
Although we provide expanded general dental care, some treatment may be not considered within the scope of general dentistry and we may refer select treatment to a specialist in order to provide the best treatment available, in which you have the option to accept the referral and pay any and all costs associated with treatment not provided by any of our locations. We reserve the right to amend these rules at any time as we see fit to optimize the services and enrollment protocol. Enrollment is not retroactive to treatment already started within our offices.
We do offer a five day grace period for patients who cannot decide on enrollment at the time of their initial exam and cleaning in which the fees paid towards that appointment can be applied towards the purchase of their membership or additional services. No refunds are available and any positive balances will be held on your account for future treatment or membership services. This membership does not apply to individuals who have dental insurance. It is your responsibility to notify us of any all changes that may affect your dental treatment or membership eligibility. We reserve the right to refuse treatment on individual family members who do not conform to our membership requirements while continuing to treat other family members who are in good standing.
In Short: We want you to have clean healthy teeth and a smile we are both proud of. If you neglect your teeth, or are disrespectful of the time we schedule for you, then not only do we not want you as a member, we don’t want you as a patient. Our goal is not to see you only when you have problems. Our goal is to prevent you from having problems and to teach you how to take care of your teeth and restore your smile to the best it can be. It will take work on your part. You’ll have to brush your teeth more than once a day, and commit to keeping them clean. You have to take care of your teeth! If you don’t want to, neither do we. You have to commit to following our suggestions, and investing in you oral health care. If we do a root canal and you don’t let us restore it with a permanent restoration and it breaks and needs to be extracted, we cannot be held accountable for your negligence. We will do our best to listen to you and understand your needs. If you are having a problem keeping your teeth clean, then just ask us to help. We want all of our patients to be healthy and happy. We promise to do our part… It’s the only way this works!
